Use the DEX CM for Community Scoring instead of creating an activity in Social Planet if you don’t need to mark off client attendance sheet, or delivering a smaller activity group that doesn’t require attendance reporting.
Go to DEX CM select on Case Files
Look up the Case File first to check if it already exists. We only want one for Community Scoring.
Select + Add DEX case file
Leave DEX Case ID blank
Choose Outlet Activity
Choose Client Attendance Profile
Enter name ie Health Literacy Program
Select Add 
Go to the Sessions tab and select + Add
In the Add Case Session
Leave Session ID Blank
Enter the Date / Time
Duration 90
Unidentified Participants add Number (optional)
Select Add 
Select all the Sessions that make up this Community Program by selecting + Add
Select on the Clients tab and select + Add a client to this case file
In the Add Client screen
Find the participant
Enter a referral source
Primary reason for Assistance
Other Reason for Assistance
Continue to add participants
Go to the Sessions tab
Select on the session eg 05 Sept 2025 11:00 AM
In the Participants select + Add
Select on the Community Score use the pencil
Enter the pre and post session score domains
Enter a Community Score Note and select Assessed By choose an option eg. practitioner
