When providing Individual Support to participants in the Settlement Engagement and Transition Support (SETS) program, the Standard Client/Community Outcomes Reporting (SCORE) tool is used for assessment.


Organizations must record SCOREs for individual clients across three key domains: 

  1. Circumstances, which measure changes in a client's overall situation (e.g., physical health, employment, housing); 

  2. Goals, which assess progress towards individual objectives (e.g., changed knowledge, skills, or behaviors) ; and

  3. Satisfaction, which captures client feedback on the service received. This comprehensive scoring helps track client outcomes throughout their service delivery.


Requirements 

You are required to report an initial and at least one subsequent SCORE for Circumstances and Goals for the majority of their clients, and Satisfaction SCOREs are generally measured at the end of service delivery.

  1. Go to Main Menu > Case File 

  2. Select Add Case File

  3. In the Open Case File window enter

    1. DEX Case ID ie. unique number

    2. Outlet Activity ie Healthy Living Centre - Community and Home Support

    3. Client Attendance Profile ie Family

    4. Name ie Housing Assistance

    5. Overview ie. Apply for Social Housing

    6. Select Add

  4. Go to the Client Profile

  5. Select Add Client 

  6. In the Add Client Window enter

    1. Select Participant ie one person

    2. Referral source ie Health agency 

    3. Primary Reason for Assistance ie Housing

    4. Other Reasons for Assistance ie. Material Wellbeing 

        7. Select Add
