Social Planet helps Community Centres deliver programs and classes for clients. It enables people to book online or for staff to book people in person. It supports Centres with their pre-accredited and accredited education services. When you’re on the Front Desk you are using Social Planet.
DEX CM
A case management system designed to help funded DSS (Department of Social Services) agencies with their scoring of individual (Client Circumstances, Goals, Satisfaction) and community (Community) outcomes using the SCORE framework.
Client Tile
The Client Tile provides a snapshot of a client's key information and allows you to quickly view their essential contact details, along with their biography, interests, and support needs.
Dashboard / Teams
Dashboard, which serves as your central hub for managing client casework. From here, you can easily see the specific Teams you've been assigned to, such as 'Employment Team,' 'Initial Team,' or 'Settlement Team'. Simply select a Team to view all the clients and case files associated with it. This helps you quickly access and manage the workload relevant to your specific responsibilities
Case files
In DEX CM, a Case File acts as a central record linking a client (or group of clients) to specific services delivered under a funded program activity. A single client can have one or more Case Files. It is designed to capture instances of service (known as sessions) that are expected to lead to a distinct outcome.
Case Files can be associated with individual client outcomes, recorded through Client Circumstances, Goals, and Satisfaction SCOREs, or with broader group/community outcomes, recorded through Community SCOREs. Each Case File contains sessions, and each session can have associated SCOREs to measure progress and achievements.
