Collecting participant data when a person first books into a course is essential information when completing government reports, grant writing and being able to conduct a program evaluation. This reduces the need for paper based non-ACFE enrolment forms and can be done from the outset of their booking. 

Setting up the Enrolment Form 

1. Go to Main Menu then select Account click on Settings 



2. On the Requests tab select the + Add  on the Supplementary Info Form pane



3.    Add Booking Supplement window enter a Name for the Form, Description, and select the Additional Fields from the  existing Profile that you want a participant to include in their booking.



4. Select Contact Enabled and Contact Required (create as a mandatory field) if you want participants to assign an emergency contact.

5. Select Has Disability Enabled if you want participants to provide any disability or medical info.   

6. Select Gender Enabled and Gender Required if you want their gender information.

7. Select First Nation Enabled and First Nation Required if you want Aboriginal and Torres Strait Islander information.

8. Select Age Enabled, then choose from Birth Date or Age Group and Age Required if you want the participant’s age.

9. Select Country of Birth and Country of Birth Required if you want to know a person’s country of origin.

10. Select Language Enabled and Language Required if you want a person’s primary language spoken at home.   

11. Then select Save 

 

Now you need to assign the Supplementary Info Form to the Activity / Course. NOTE to activate the form the activity must be assigned to Simple Booking 

 

1, Go to Main Menu then go to Activities select Current 

2. Enter the Activity Name in the Search Field when the Activity Tile appears select Edit then go to the Settings tab


3. On the Booking Process Type select Simple Booking 

4. In the Booking Settings pane select Edit then scroll down to the bottom and select Include supplementary information form use the drop down and select Name of the Supplementary Information Form you have created.




 Press Save 

5. When a client completes an online booking they're automatically taken to the supplementary form to complete. Information they enter will update their profile. 



For backend bookings staff can email the form to participants 


6. In the booking request complete page, select Send supplementary info email 



7. The Supplementary Information window will appear then select Send
